A complete care record contains information gathered from birth and throughout the client's lifetime (whatever the setting or the professional discipline) in a single integrated and longitudinal record. Clinicians can view historic and current client care and can see who else is or has been involved in the client's care (subject to stringent security options).
A highly configurable, multi-disciplinary care management module provides the means to create and monitor care pathways, care plans and assessments. The care planning functionality is designed with clinical governance in mind. Packages of interventions are built to address a specific need (e.g., leg ulcers) using the core care plan templates. Users build the patient care plan by importing and customising the pre-built packages of care. Target outcomes, actual outcomes and variances are recorded for effective clinical governance reporting.
Ascribe's community systems fully supports the Single Assessment Process for Older Persons. Individual assessment components are be designed (using the template builder) to match local processes, which are then built into a variety of combined web forms specific to a professional group or a particular stage in the assessment process. A series of customisable report views, allow show a snapshot of up-to-date information from all stages of the assessment process.
Contacts may be scheduled in advance using the staff organiser module. In addition, the system supports facility booking, management of non-contact time, mileage analysis and timesheet production.
Contact recording is closely integrated with the diary management and care planning to minimise effort in data entry. Pre-scheduled diary events and planned interventions may be quickly converted into historical contact records with a few mouse clicks.
The child health module provides automated scheduling for child vaccinations and immunisations, and examinations. Its design is underpinned by the Child Health Informatics Consortium’s (CHIC) specification. The suite of pre-built reports includes COVER statistics and SureStart reports.
Microsoft Word integration allows local administrators to define standardised letter/document templates including data fields, which may be quickly generated and printed by users, then saved in the database as part of the patient record. For example, our customers use Word documents to produce letters and provide easy-to-read printouts of care plans and assessments for patients. Scanned documents may be also be saved into the patient record.
The system may be configured to send alerts to users, giving notice of forthcoming review dates and other significant events relating to their patients. The alerts are displayed as a task list whenever the user logs in. Alerts remain in the user’s task list until they dismiss them (at which point they are archived and the date and time of dismissal are logged in the audit trail).

A suite of built-in reports is provided, requiring no local query writing. These include operational reports, e.g., overdue contacts, activity by location, and national returns. In addition, industry-standard third-party products such as Crystal Reports are used to interrogate data to meet local reporting requirements (full training is provided), or data may be offloaded into a data warehouse.

Improved Financial Control
ADC holds financial information about debtors and patients. Standard fee schedules can be created by finance staff and linked to debtors to create financial programs. These can then be allocated to the clients to specify the type of service, the appointment schedule, authorisation limits and rules for travel, consumables and equipment.
The accounts receivable cycle is reduced by allowing charges for services to be split between multiple debtors and the client ensuring invoices are correct and complete.
Better information at the point of care
A single client record holds all the information for all the services associated with the client such as nursing, allied health, personal care, home maintenance, transport, delivered meals and respite care. Field staff that are equipped with mobile computers are able to manage their appointment and record information about clients on the spot. The system alerts clinical staff to any medical conditions or special needs of a patient and enables them to attach notes.
